Unable to insert image to the post

Namaste

The "insert image" and "Manage attachment" functions are not working as desired. Upon clicking the link, although it opens up a page to upload image I am unable to see the uploaded image.

In the "Manage attachment" window I observed a blank thumbnail ShivaFan.jpg. I don't remember uploading it ever before. Neither I am able to use it nor to delete. I use Firefox 47. I faced same problem in chrome as well.

I wanted to know if these functions are disabled? Has any one tried it recently? I saw images in Yajvan's recent threads.
